Are you looking for a family doctor or primary care practitioner?

The Health Care Connect program will help you find doctors and nurse practitioners who are taking new patients in your community.

If you don't have a health care provider now, you can register for the Health Care Connect program online or call 1-800-445-1822.

Before you register

There are some things that you’ll need to have before you can register with the Health Care Connect program.

You cannot be enrolled with a family doctor

This program is for those who do not currently have a family doctor or primary care provider. Call ServiceOntario at 1-866-532-3161 or TTY 1-800-387-5559.

Have your health card number ready

You must have a valid Ontario health number, which is on your health card. You’ll need your health number to join the Health Care Connect program.

Update your address

Make sure that the address on your health card is up-to-date. You can update your health card with your new address and/or phone number.
