Patient Records

If you would like a copy of your own patient records, such as your medical chart, please consult with your doctor or the Health Records Department at the Brockville General Hospital (BGH) main site.

Consent forms

If you would like a copy of another patient's records, you will need them to sign a consent form. Please have the completed form printed and return it to the Health Records Department. A patient's consent is required to disclose any personal health information to you.
