Resources for your total joint replacement journey can be found on this page.

Bundled Care

About Bundled Care

In 2015, the Ministry of Health (MOH) introduced a new funding model to aid with Total Joint Replacements (TJR). This new integrated funding model was designed as a way to continue towards the development of an Ontario health system with a goal of ‘putting patients first’.

Their vision was to promote greater integration in health care dealings and streamline the process of TJR with the overall goals of delivering high quality and efficient care while improving patient outcomes and experiences. This new pathway allows for seamless transition between types of care thereby creating less confusion for the patient and their family. It is the same team working with you from the very start all the way to the end.

Beginning in the fall of 2020, the Brockville General Hospital will be providing Bundled Care for elective hip and knee replacements. This single payment method will cover the cost of all needs for each patient, from surgery to discharge.

This new model will help integration of care between individual hospitals and the communities with which they serve. The BGH will be responsible for ensuring all payments to qualifying partner and non-partner organizations that offer care related to recovery and rehabilitation following an elective hip or knee replacement. This single payment will cover inpatient, outpatient, and rehabilitative services.

Brockville General Hospital has been approved by the MOH to participate in this Bundled Care expansion and our team is excited to continue working with you, the patient, to make this experience the best it can possibly be.

Associates

Brockville General has entered into a formal agreement with Ontario Health atHome South East.

Non-associates

Brockville General will also be working with non-partners, at the request of patients, and following a referral from Brockville General, they will be able to provide this same service.

Non-Partners include:

  • Organizations that provide Ontario government-funded home care services.
  • Any Ontario hospital-operated inpatient rehab unit or hospital-based outpatient rehabilitation.
  • Any Ontario government-funded community outpatient rehabilitation provider such as publicly funded physiotherapy clinics.
