Requesting Information
If you would like to make a formal request for information at Brockville General Hospital that you cannot access through an informal request or through our website, you can submit a written request through the Freedom of Information and Protection of Privacy Act (FIPPA).
Written request
You may request access to information by making a written request through our Freedom of Information (FOI) Coordinator. While writing your request, remember that the more specific your request, the quicker and more accurately we can respond.
Please include the following necessary information in your request:
- Date of request
- Identification of the specific record(s) to which you are requesting access
- Statement that you are making this request through FIPPA
- $5 application fee
- Your contact information
- An original signature of the requester
FIPPA requests submitted by email cannot be accepted, since the legislation requires that you sign your requests with an original signature.
Application fee
Your $5 application fee helps pay for the processing and work required to retrieve the information. If the total cost of completing your request exceeds $100, you will be provided with a fee estimate. You will be required to pay 50% of the total estimate before processing begins in this case.
Submitting your request
Submit your request to the Brockville General Hospital FOI Coordinator at our Charles Street main site or mail your request with your payment. You assume all risk of sending money through the mail if the payment is lost or stolen. Your request can be mailed to:

Reviewing your request
The FOI office will review your request and send an acknowledgement letter and any estimate of further fees after receiving your request. We try our best to resolve your request within 30 days, but we may notify you if a time extension is needed.
Once a determination has been made, we will send a decision letter to you, outlining the details of the decision. This can include possible exemptions, a calculation of any incurred fees and, if applicable, a schedule of disclosure. Records with information exemptions may be withheld or redacted (blacked out) and the decision letter will explain the applied exemptions. You may be asked to present a piece of photo identification if the records contain personal information about yourself.
Appealing the decision
You can appeal all decisions made by Brockville General Hospital during the process to the Information and Privacy Commissioner (IPC) of Ontario within 30 days of the date of the hospital's decision letter. Contact the IPC by email, at 416-326-3333, or toll-free at 1-800-387-0073.
