Brockville@Home brings hospital-to-home recovery support to patients
A new program helping patients continue their recovery at home is showing early signs of success, thanks to a close partnership between Brockville General and SE Health. Launched in March, Brockville@Home is designed to help people regain independence after a hospital stay, while staying in the place they’re the most comfortable: their own home.
“Brockville@Home gives patients an eight - or 16-week option to keep progressing toward their restorative goals from the comfort of home,” explains Samantha, Patient Flow Coordinator, Brockville@Home Team Lead. “We start with the supports people need at a higher intensity, then reduce services over time as they build towards independence. Our goal with patients is clear, support their autonomy, build their confidence and help participants move steadily toward self-care.”
A key ingredient is the program’s delivery model with SE Health’s dedicated Brockville@Home team, which reviews every enrolled participant each day.
“Christine, SE Health’s Transitional Care Lead, works on-site at the hospital,” Samantha continued. “She completes initial assessments of patients, creates care plans, schedules Brockville@Home team visits, orders equipment, and meets with the team daily.”
Those daily check-ins help the team share updates, adjust services quickly, adding new supports when needed or reducing those no longer required, and keep everyone aligned on the same goals.
“Working for Brockville@Home as a nurse, I see the health benefits of people being supported in the comfort of one's home, a familiar environment supported by a multidisciplinary team who empower the patient along their care journey,” added Christine, SE Health Transitional Care Lead.
Once a participant is enrolled in the program, they can receive a mix of in-home supports tailored to their needs, including nursing, personal support, physiotherapy, occupational therapy, social work, dietitian, and speech language pathology. Every participant is connected to a nurse for the duration of the program.
Since launching in March, Brockville@Home has enrolled 26 patients as of May. While the program is still in its early days, feedback has been encouraging. One participant shared that they don't believe they would be recovering—or moving toward self-care—as quickly without Brockville@Home’s support.
“It’s exciting to now have this program available at our hospital,” said Jackie, Program Director, Mental Health, Allied Health, and Clinical Services. “Home is where many people feel safest, and with the right supports in place, patients can recover faster and regain their confidence.”
