Request Your Own Records

You have the right to request access to your personal health information held by Brockville General.

Examples may include:

  • Hospital visit records
  • Discharge summaries
  • Operative reports
  • Diagnostic imaging reports
  • Laboratory reports

To request a copy of your records, please contact the Health Records Department at Brockville General's main site. 

You may be asked to provide identification and complete an authorization form.

Request Records for Someone Else

A patient's personal health information cannot be released to another person without proper authorization, except where permitted by law.

If you are requesting records on behalf of another individual, a signed consent form is required.

Consent forms

To authorize the release of personal health information to another individual, please complete the Consent to Disclose Personal Health Information form. Completed forms can be submitted to the Health Records Department.

If you would like a copy of your own patient records, such as your medical chart, please consult with your doctor or the Health Records Department at the Brockville General's main site.

Consent forms

If you would like a copy of another patient's records, you will need them to sign a consent form. Please have the completed form printed and return it to the Health Records Department. A patient's consent is required to disclose any personal health information to you.
