You may be eligible for cardiac rehabilitation if you are recovering from:

Programs are offered in hospital or from home and are tailored to your individual needs. Depending on your goals and recovery, you may participate in a 6- or 12-week program.

What to Expect

After your referral is received, a member of our team will contact you to arrange an initial assessment. During this visit, we will review your health history, discuss your recovery goals, and develop a personalized rehabilitation plan.

Programs are tailored to your individual needs and may be completed at the hospital or from home. Depending on your goals and recovery, you may participate in either a 6- or 12-week program.

How to Join

A referral from your healthcare provider is required. Services are provided at no cost to eligible patients through generous community support and healthcare funding.
