Community Stroke Rehabilitation

Brockville General has collaborated with the Stroke Network of Southeastern Ontario to bring Brockville’s own Community Stroke Rehabilitation (CSR) clinic to life. If you have had a stroke in the last 12 months, you may be eligible for program enrollment.

The CSR program offers intensive Physiotherapy, Occupational Therapy, Speech Therapy, and Social Work services to support individuals in their recovery journey. This program consists of clinic therapy, community therapy, and virtual therapy services. Our team will also provide you and your family with the support and education needed to manage life after having a stroke.

A doctor's referral is required to register in the CSR program. Our services are free to eligible patients, thanks to the generous funding provided by our local community. 

We are located on the ground floor of Brockville General's Charles Street main site. For more information about the program, please call 613-345-5649, extension 5XXXX. 

Our CSR clinic is open Monday to Thursday from 8:00 a.m. to 4:00 p.m.

Rehabilitation Services

Our multidisciplinary team will work closely with patients, families, and caregivers to support recovery every step of the way. Services are tailored to the individuals needs and may include one or more of the following professions:

  • Physiotherapist
  • Occupational Therapist
  • Speech Language Pathologist
  • Social Worker
  • Communication Disorders Assistant
  • Rehabilitation Assistant
